Health Education, Rooted in Community

Our Health Promotion Program, led by trained health educators, helps people learn how to care for their overall health and feel their best.

We offer one-on-one support, group classes, wellness activities, community events, and health campaigns. These services help people prevent and manage health conditions, find healthy food, stay active, and build healthy habits that fit into daily life.

Services We Offer

Our health educators help people living with long-term health conditions, as well as their families and loved ones. They can provide health education, help people find services, and support them as they take steps toward better health. Our goal is to make sure every person feels informed, supported, and able to make healthy choices.

Group and individual educational sessions on preventing or managing chronic conditions, including prediabetes, diabetes, obesity, and hypertension.

Support for daily self care, providing guidance on how to follow their provider's recommendations and teaching skills to keep track of their health.

Coaching lifestyle changes, with tools to apply skills at home.

Support to understand health information in clear, accessible language aiming to promote health behavior change.

Guidance in navigating healthcare, including primary care and prevention services.

Navigation to social support community resources.

Physical activity and movement-based activities.

Healthy weight support, including goal setting and health education.

Wellness practices that can be used every day.

Cooking demonstrations featuring nutritious, affordable, culturally familiar food recipes.

Support identifying healthy food options in local farmers markets and community food banks.

Guided visits to markets and stores.

Information on food access programs, like community food banks, and how to use them.

Education on environmental factors that affect health at home and in the community.

Wellness activities in community spaces, such as parks and recreation areas.

Health fairs, community talks, and wellness events.

Peer-led education sessions facilitated by Promotores de Salud.

Workshops focused on skills-building, prevention, and community action.

Each session is active, respectful of each person’s culture, and based on the real needs and experiences of our community.
