I Need Assistance

We help patients apply for insurance and offer payment assistance programs to make care affordable for those who qualify.

Step 1. Make an Appointment

Step 2. Gather your documents

To apply for assistance, please bring the following to your appointment.

Please bring 1 of the following:

  • Government-issued photo ID or driver’s license
  • Passport or consular ID
  • Work or school ID
  • Birth certificate (for children)

Don’t have an ID? Come anyway — our team can help.

Please bring 1 of the following (dated within the last 60 days):

  • Utility bill (gas, electric, water, phone)
  • Lease agreement or rent receipt
  • Bank or credit card statement
  • Official mail with your name and address

Please bring 1 of the following:

  • 4 paystubs if paid weekly
  • 2 paystubs if paid biweekly
  • 1 paystub if paid monthly
  • Most recent tax return (1040)
  • Employer income letter
  • If unemployed, please provide a letter of financial support

Please bring:

  • List of current medications
  • Your medical records & vaccine records
  • List of questions for your provider

Step 3. Review Costs Based on Income & Household Size

Once approved, your discount lasts one year unless your income or household changes.

No. Once you are approved, your discount lasts one year. You only need to reapply if your household size or income changes before then, or when the year is up.
